Initial Application

This is the initial application for Disability Dogs to review you needs and determine if you are a viable candidate. This may be returned via email or text. Please COMPLETE ALL questions.

Medical Disclosures

This is file is to be printed out. It must be completed by the medical professional that the oversees the disability the dog is for. It may be scanned and returned, however the original is required prior to receiving the dog.

Owner Trainer Consideration

If you have a dog you want evaluated for the Owner Trainer evaluation please complete this form and return it prior to the dog being evaluated

Am I allowed to have other pets/animals in my home?

Yes and No, we allow you to own other dogs, cats, etc. However, we do require that you sign a waiver understanding that the Service Dog is not a pet and recognize that there may be conflicts with introducing a new animal.

How long is the wait list?

We do not currently have a wait list for approved applicants to receive a service dog.  Approval time varies due to application review approval process, and specialized training needs of the dog which can take approx. 2-6 months depending on how quickly we receive all required materials.

Are there other requirements or restrictions for applying?

  1. For anyone (Veteran or civilian) who wants to be considered for a PTSD dog, you must:

  • be at least twenty-one (21) years of age

  • have an official PTSD diagnosis

have a minimum of one (1) year of recent mental health care pertaining to the PTSD diagnosis


Our standard age requirement is twelve (12) years old for mobility and medical response (seizure response and diabetes alert).


Limited availability if you have multiple complex mental health diagnoses such as personality disorder, autism, etc.

Not all individuals are qualified for a dog to be donated. 

What about travel and expenses?

The costs for travel expenses are separate from the amount paid for the service dog itself. You must be able and willing to travel to our Poplarville, MS or Gainesville, FL campus (depending on SD type) for team training for a minimum of one (1) week at your own expense.
